Verordnungsverhalten in einem psychiatrischen Krankenhaus unter dem Aspekt des Off-Label-Use
- Die Arbeit untersucht den Off-Label-Use von Psychopharmaka in einem psychiatrischen Krankenhaus vor und nach einem Grundsatzurteil des Bundessozialgerichts von 2002. Zwei randomisierte Stichproben aus allen Behandlungen der Jahre 2001-2002 und 2003-2004 \((n_{1}=125; n_{2}=126)\) wurden untersucht. Die Psychopharmaka-Verordnungen wurden als \(\textit {eindeutig off-label}\), \(\textit {uneindeutig off-label}\) und \(\it on-label\) eingeordnet. 2001-2002 waren 20% aller 584 Verordnungen \(\it eindeutig\), 19% \(\textit {uneindeutig off-label}\). 2003-2004 waren es 21% bzw. 26% von 698. In jeweils >50% der Behandlungen war mindestens eine Verordnung off-label. Im Gegensatz zu Psychopharmaka- und Diagnosegruppen zeigten sich hinsichtlich Geschlecht und Behandlungsalter keine signifikanten Unterschiede. In der Entlassmedikation war der Off-label-Anteil signifikant erhöht. Es zeigt sich also ein häufiger Off-Label-Use, der Daten aus anderen Ländern entspricht. Angesichts der möglichen negativen Konsequenzen ist dies ein noch ungelöstes Problem.
- This study examines off-label prescriptions in a German psychiatric hospital before and after a judgement of general principle of the Federal Social Court in 2002. Two randomized samples \((n_{1}=125; n_{2}=126)\) of all patients treated in the years 2001-2002 and 2003-2004 respectively were examined. All prescriptions of psychotropic drugs were categorized as \(\textit {clearly off-label}\), \(\textit {probably off-label}\) or \(\it on-label\). In 2001-2002 20% of all 584 prescriptions of psychotropic drugs were clearly, 19% probably off-label. In 2003-2004 the numbers were 21% and 26% of 698 respectively. In more than 50% of all cases at least one prescription was off-label. There was no significant influence of sex and patients' age on off-label prescribing. Drugs prescribed on discharge were significantly more often prescribed off-label. In accordance with data from other countries the results show a high prevalence of off-label-use. Although measures have been taken to solve the ensuing problems, further solutions are needed.
